A penile prosthesis (PP) is an internal medical device designed to restore dependable erectile function for patients whose condition cannot be adequately treated with medications or less invasive therapies. While commonly used for severe or treatment-resistant erectile dysfunction (ED), penile prostheses are also an effective solution for other conditions that impair erectile function, including Peyronie’s disease, nerve injury, pelvic or prostate surgery, vascular disease, and penile trauma. These conditions can significantly affect intimacy, confidence, and overall quality of life. There are two primary types of penile prostheses: inflatable and malleable. Inflatable devices are the most commonly used and offer the most natural appearance and function. They allow the penis to remain fully flaccid when not in use and provide rigidity only when desired. The inflatable system consists of three components: cylinders placed within the penis, a fluid reservoir positioned in the lower abdomen, and a pump discreetly located in the scrotum. When activated, the pump transfers fluid into the cylinders to create an erection; pressing the release valve returns the penis to a flaccid state.
Malleable prostheses consist of two bendable cylinders that remain semi-rigid at all times. The penis can be manually positioned upward for intercourse and downward for daily activities. While simpler in design, this option may be appropriate for select patients based on anatomy, medical history, or personal preference.
